Description
The Licensed Social Worker (LSW) is responsible for providing medically related social services to residents and their families to attain or maintain the highest practicable physical, mental, emotional, and psychosocial well-being in accordance with resident-centered care principles. The Social Worker serves as an advocate for resident rights, dignity, self-determination, and quality of life while ensuring compliance with Minnesota Department of Health (MDH) regulations, Centers for Medicare & Medicaid Services (CMS) Requirements of Participation, and all applicable federal, state, and local regulations.
The Social Worker collaborates with residents, families, physicians, community agencies, Veterans Affairs representatives, Medicare and Medicaid resources, and interdisciplinary team members to coordinate services, facilitate care transitions, support psychosocial well-being, and assist with financial and payer-related concerns.

Minnesota Department of Health nursing home regulations
CMS Requirements of Participation (42 CFR 483)
Resident Rights regulations
Minnesota Vulnerable Adult Act and suspicion of a crime
HIPAA privacy and confidentiality requirements
Medicare skilled nursing facility coverage requirements
Minnesota Medical Assistance (Medicaid) eligibility processes
Managed care and insurance authorization processes
Veterans Affairs benefits and long-term care programs
MDS and care planning requirements
Discharge planning regulations
Abuse prevention and mandatory reporting requirements
Dementia care and behavioral health interventions
Quality Assurance and Performance Improvement (QAPI) principles
MINIMUM QUALIFICATIONS
Bachelor’s degree in social work (BSW) from an accredited college or university required. Master’s degree in social work (MSW) preferred.
Current Minnesota Social Work license in good standing required.
Minimum one year of experience in long-term care, skilled nursing, healthcare, rehabilitation, hospice, or related setting preferred.
Working knowledge of Medicare, Medicaid, and long-term care reimbursement systems preferred.
Experience with electronic health records and care planning systems preferred.
Ability to maintain confidentiality and exercise professional judgment.
Strong written, verbal, organizational, and interpersonal communication skills.
LICENSES AND CERTIFICATIONS
Current Minnesota Social Work License (LSW).
Current background study clearance as required by Minnesota Department of Human Services.
TB screening requirements as established by facility policy.
