Description

Plan, schedule, and complete MDS assessments and submissions.  Lead interdisciplinary care planning and documentation review.  Identify and address changes in resident conditions.  Monitor Medicare eligibility and documentation.  Ensure compliance with state and federal guidelines.  Support Quality Assurance and Performance Improvement (QAPI) efforts.  Educate and support clinical staff on the MDS process.

Requirements

Current RN license in good standing.  Prior MDS/RAI experience preferred (will train the right candidate).  Knowledge of state and federal long-term care regulations.  Strong assessment, communication, and documentation skills.  Passion for person-centered care and teamwork.
